Customer Facing Forms
Customer Facing Forms let you collect and verify entity information by sending a secure form directly to your customer. Forms are sent from the entity page, completed by the recipient through a guided flow built with Signicat, and returned to Strise where you can review the responses — and, for companies, compare them against the data Strise already holds.

Where to find it
The Forms section appears as its own card on both company and private person entity pages, for teams with the Forms module enabled. Each row shows the form template name, the date it was sent or completed, and a status badge. A row menu (⋯) exposes the actions available for that status.
If the section isn’t visible, the module isn’t enabled for your team — contact your account manager or support@strise.ai.
Sending a form
Click Send form to open the Send form dialog and fill in:

| Email Address | Where the secure link is sent. |
Click Send form. The recipient receives an email with a secure link and completes the form on their own device. The form appears in the section with a Pending status straight away and updates automatically as the recipient progresses.
Statuses
| Status | What it means |
|---|---|
| Pending | The form has been sent and the recipient hasn’t opened it yet. |
| In progress | The recipient has opened the form and is filling it in. |
| Completed | The recipient submitted the form and the response is available. |
| Inconclusive | The recipient submitted the form, but document verification could not be conclusively completed. The response and submitted files are still available. |
| Failed | The form could not be delivered or processed. |
| Cancelled | The form was cancelled before completion. |
The section refreshes on its own while any form is pending or in progress, so statuses stay current without a manual reload.
Managing forms
From a row’s ⋯ menu, while a form is pending or in progress, you can:
- Copy link — copy the recipient’s secure link to share it directly.
- Cancel — cancel a form that’s no longer needed.
Reviewing responses
When a form is Completed or Inconclusive, click the row to open the details drawer. It shows when the form was sent and completed, and lists the files the recipient submitted, ready to download. For an inconclusive form, the drawer also shows the reason document verification couldn’t be conclusively completed.
Comparison view (companies)
For companies, a completed response can be compared against the data Strise already holds. Open Compare on a submitted document to see a field-by-field comparison:
- If everything lines up, Strise tells you the information from the document correlates with the information in Strise.
- Where values differ, each discrepancy is listed with the document value next to the Strise value, so you can act on exactly what changed.
This reuses the same comparison engine as Registry Check.
Templates
Form templates are configured for your team and powered by Strise’s forms partner, Signicat. A template defines which fields to collect and which documents to request; when you send a form you simply pick the template, name the recipient, and enter their email. Forms can be white-labeled and pre-filled with Strise data so the recipient only has to confirm or correct what’s already known.
Sending via the Connect API
Forms can also be triggered programmatically through the Connect API, for teams automating onboarding or remediation workflows. See the API documentation for details.
